What Should I Expect at My First Physical Therapy Visit?

On your first visit, the physical therapist will perform a thorough evaluation lasting approximately 45-­60 minutes that includes history, postural assessment, neurological screening, and tests and measures to gain a more specific understanding of your condition. Your physical therapist will use the information obtained from your evaluation to formulate a clinical judgment as to your prognosis and appropriate treatment intervention. A rehabilitation program will be developed based upon the problems identified and the individual’s personal goals.

Please wear loose­-fitting, comfortable clothing so you can expose the area that we will be evaluating and treating. Please arrive 10-15 minutes early ; for your initial evaluation.

West Town (1023 N Ashland Ave) Parking: Most of the parking in the area is permitted, but there is Pay-to-Park along Ashland. There is free parking one block south on Augusta Blvd or two blocks south on W Walton St. There is also a little lot next door, but that is not ours! Do not park there, at risk of being towed.

Roscoe Village (1900 W Belmont Ave) Parking: Free street parking on Belmont & Wolcott as well as surrounding neighborhood.

Things to do before your first visit:

  1. Book Appointment
  2. Complete Intake Information (in confirmation email)
  3. Complete and sign paperwork (sent via email)
  4. Send photo ID & insurance card (admin@westtownpt.com)
  5. Send physical therapy prescription/referral from your doctor (if you have one
